How to Add Custom Fields to Forms

We know there’s no such thing as a one-size-fits-all form.
For that reason we’ve built in two ways for you to add additional fields.
Option #1: Add Additional “Base Fields”

Option #1: Add Additional “Base Fields”
“Base Fields” are pre-configured fields that contain information that can be shared throughout the rest of Sportamingo.
For example.. an athlete’s birthday, school, grade/grad year, etc.
There’s no reason to constantly re-create those fields so we gave you the option to simply check a box and have them dropped into your form.
As an added bonus, when those fields are completed by families they will automatically update a member’s profile information.
Option #2: Adding Custom Fields
Sportamingo also supports the ability for you to request information specific to your needs.
Using a drag-and-drop editor you are able to add single-line text inputs, multi-line text boxes, dropdown lists, radio buttons and check boxes.
You also have the ability to add text directly onto the form as a heading or paragraph text, giving you a great deal of flexibility in terms of what you’re able to create.
Step-by-Step Guide + Screenshots
Step #1: Login to Sportamingo
If you are not logged in yet you can click the "Login" button in the top right.
Step #2: Open Club Admin --> Forms
You can find this dropdown menu at the top of the page after logging in. Select the appropriate club and click on "Forms" in the dropdown.
Step #3 - Click "Create" Button
This will open a popup form that will give you two options: "Create From Scratch" (default) or "Create From Library"
The rest of this guide will continue as though we are creating a new form from scratch, just know that you can later save forms as a template and use them to quickly re-create forms and tweak settings, instead of re-inventing the wheel every time.
Step #4 - Select Form Type
This will default to "Registration - Junior Athlete" but you can switch this to "Team Registration - Junior Athletes" if need be.
Step #5 - Enter a Form Label
As you type you will notice that the "Form URL Slug" changes as well. Don't be concerned if this ends up matching a URL you previously used - we can change this later.
Step #6 (optional) - Set Form Slug
Please keep in mind that every form must have a unique URL. If you try to create a form using a URL that is already taken we will block it and give you a heads up.
You are not required to use the auto-suggested URL - if you would like to customize it you can click on the "Form URL Slug" field and modify that value.
Step #7- Click "Create Form" Button
Once we are done building the form we'll re-direct you to the form settings page so you can customize further.
