Creating a Form in Sportamingo

This guide is part of the article series "Mastering Forms in Sportamingo"

There are currently two supported form types:

#1: Registration - Junior Athletes

-and-

#2 - Team Registration - Junior Athletes

Junior athlete registration is ideal for events like tryouts, open gym signups, summer camps and the sort... anything where athletes are signing up as an individual.

Team registration should only be used when running a league or tournament.

How do the form types differ?

Both forms will require six base fields: athlete first and last name; and parent first name, last name, email address and phone number.

However, with team registration selected, members will first have to choose whether they are creating a new team or joining an existing team.

The other difference is that, with team registration, only the team captains are billed the team fee. Team participants will not be charged while joining a team unless they select options in your custom fields that increment the cost, but we'll discuss that in greater detail later.

What about adult registration?

We plan to support registration for adult athletes in the future but our current focus right now is on juniors. Once we get our core product completed we'll circle back and expand into this area.

Step #3 - Click "Create" Button

This will open a popup form that will give you two options: "Create From Scratch" (default) or "Create From Library"

The rest of this guide will continue as though we are creating a new form from scratch, just know that you can later save forms as a template and use them to quickly re-create forms and tweak settings, instead of re-inventing the wheel every time.

Step #4 - Select Form Type

This will default to "Registration - Junior Athlete" but you can switch this to "Team Registration - Junior Athletes" if need be.

Step #5 - Enter a Form Label

As you type you will notice that the "Form URL Slug" changes as well. Don't be concerned if this ends up matching a URL you previously used - we can change this later.

Step #6 (optional) - Set Form Slug

Please keep in mind that every form must have a unique URL. If you try to create a form using a URL that is already taken we will block it and give you a heads up.

You are not required to use the auto-suggested URL - if you would like to customize it you can click on the "Form URL Slug" field and modify that value.

Step #7- Click "Create Form" Button

Once we are done building the form we'll re-direct you to the form settings page so you can customize further.
